Community Support

We are grateful for the opportunity to support our community in times of need. Listed below are the partnership, education, and outreach services that we provide to local schools and community organizations, free of charge.

Crisis Support

The Community Hospice provides support to neighborhoods and communities where sudden traumatic death occurs.

When communities are faced with a death, the Community Hospice bereavement team is available to work with them to organize and implement their crisis plan.

Whether the death is anticipated or sudden, the Community Hospice bereavement team will assist your school, organization, or community in organizing and implementing a plan of action for those impacted. These action plans may include grief education to leadership/administration teams, immediate support to those impacted by the loss, identifying at-risk community members and providing interventions or referrals to suitable professional providers, and assisting with plans to memorialize the person who died.

Please call 518-724-0200 for more information.

**If you're with a school, business, or organization that needs to reach us nights or weekends to set up crisis support in response to a sudden death, call (518) 724-0200 and ask for crisis bereavement.**

Community Partnership

We partner with local schools and community organizations to provide support services to anyone that is grieving the death of a family member or friend and educational opportunities for those interested in learning more about the bereavement process. These services may include on-site individual counseling and support groups, grief and loss educational trainings, and workshops. Educational materials and handouts also available. These services are free of charge.

Please call (518) 724-0200 for more information.
